Isabeau Jacobsen
Principal Occupational Therapist
Isabeau is an Occupational Therapist with a strong clinical focus on capacity building, functional independence, and helping people work towards goals that are meaningful to their everyday lives.
Her approach combines participant goals, current evidence, and practical solutions that can be implemented within the environments where people actually live, work, and participate.
Why I Chose Occupational Therapy
Isabeau became an Occupational Therapist because she wanted to help people achieve goals they may have spent much of their lives trying to reach, sometimes without recognising the skills they already have or having the right support to develop them.
What continues to motivate her is seeing people develop the skills and strategies needed to achieve meaningful goals and live their lives with greater dignity.
For Isabeau, good occupational therapy should help people understand their strengths and difficulties, build skills where possible, and develop practical compensatory strategies where needed to support a fulfilling life.
A Practical and Capacity-Building Approach
-
Isabeau begins with the participant’s goals, strengths, learning capacity, and what a meaningful life looks like for them.
-
Her primary focus is capacity building: identifying meaningful goals, determining actionable steps, developing the skills required, and problem-solving barriers that arise along the way.
-
Intervention combines current evidence with practical solutions that account for the environments, resources, supports, and circumstances in which the participant actually lives.
-
Isabeau places strong importance on ensuring her own clinical skills are appropriate for the participant’s needs. Where another clinician or service is better placed to help, she aims to support the person in identifying a more suitable pathway.
Clinical Practice
Isabeau holds a Bachelor of Occupational Therapy and is a Driving Trained Occupational Therapist. As Principal Occupational Therapist, she provides clinical leadership across PCT’s occupational therapy services while maintaining an active clinical caseload.
Her areas of particular interest and strength include:
Capacity-building occupational therapy
Functional Capacity Assessments
Home and Living Assessments
Assistive Technology Assessments
Occupational Therapy Driving Assessments
Functional skill development
Complex and higher-support-needs presentations
Clinical report writing
Multidisciplinary collaboration
Clinical supervision and quality assurance
Her strongest clinical focus is capacity building, particularly helping participants identify meaningful goals, develop actionable pathways towards them, and problem-solve the challenges that arise along the way.
Quality Outcomes and Quality Service
Isabeau wants PCT’s occupational therapy services to be known for both the quality of the support people receive and the outcomes that result from it.
She wants participants, families, and support teams to feel heard, understood, and valued, and to have confidence that the support being provided has a clear purpose.
Where PCT can help, the goal is to work towards meaningful progress. Where another solution is more appropriate, quality service also means recognising that and helping the participant find the right support.
Occupational therapy should help people understand their abilities, develop practical skills, overcome barriers, and work towards a life that is meaningful and fulfilling to them.
